Hard Flaccid Syndrome is characterised by a persistent semi-erection, loss of penile rigidity, sometimes with a cold and soft glans, with the inability to achieve or maintain an erection. Sensory changes such as coldness and numbness can occur in the penis.

HF usually presents with a sudden onset of erectile dysfunction and in some cases a loss of nocturnal and morning maintenance erections. Even in the absence of sexual arousal.
